
Office Policies & Philosophy
Appointment & Scheduling Policy
Your appointment is very important to us, and the appointments we book are reserved specifically for you. We understand that sometimes schedule adjustments are necessary, and life can be unpredictable. Our office policies are outlined below:
1. We kindly request at least 48 hours notice for appointment cancellations and/or rescheduling.
2. Repeated missed appointments or late cancellations may result in dismissal as a patient.
3. Our office REQUIRES appointment confirmation - via text, phone, or email correspondence - to maintain your reservation. If an appointment is not confirmed within 48hrs of the appointment date, it may be cancelled by our office. Please ensure that your contact information and communication preferences are correct in your form submission to avoid any inconveniences.
4. If you arrive more than 10 minutes late to your appointment we will do everything we can to accommodate. However, we deem it necessary to reschedule if we cannot render care in the shortened amount of time.
Financial Policy
Payment in full for your treatment is due when services are rendered. Acceptable forms of payment include cash, Visa, Master Card, American Express, Discover, and assigned insurance benefits.
We offer payment plans via third-party financing.
Payments declined due to insufficient funds will receive a call and/or text within 1-2 business days. If no response or arrangement can be made, a fee of $25.00 will be applied to the account.
Dental Insurance Policy
Please keep in mind that our office verifies insurance eligibility and submits insurance claims as a courtesy to our patients. However, we are not responsible for how your insurance company handles claims or for what benefits they pay. Our office will provide the best ESTIMATE possible for services that are included under your dental plan.
In the event that insurance reimburses MORE than our estimate, you and/or your insurance will be reimbursed by our office.
In the event that insurance reimburses LESS than our estimate, you (the patient) are responsible for the remaining balance.
Changes to Your Insurance
Most importantly, please keep us informed of any insurance changes, regardless of how insignificant the change may seem. Many times benefits change yearly as your contract renews or if you have a change in employment (even if it is the same insurance company). Depending upon of how you acquired your dental insurance coverage, it is always advisable to check with your employer’s human resources department, your insurance carrier, or the representative/company who sold you the policy to verify any changes to your benefits.
